Federal Tax Identification This is required for departments that are applying for the Volunteer Fire Assistance Grant
Population Base Served In Your PRIMARY Protection Area This is required for all applicants. ( inflated numbers will only delay processing of your application)
Size Of PRIMARY Protection Area In Square Miles This is required for all applicants. ( again, inflated numbers will only delay processing of your application)
Insurance Services Office (ISO) rating This is required for departments that are applying for the Volunteer Fire Assistance Grant
List Public Lands In Your PRIMARY Protection Area Greater Than 500 Acres (State Forest and Parks, County Forest, or Federal Forest, Wildlife areas, Wilderness) The number of acres protected are required and must be accurate. If you are unsure how many acres you are responsible for contact the property manager for assistance.
Briefly describe the purpose of this proposal. (What are you upgrading or adding to your department) Example: We would like to purchase new hose for our brush truck to replace the existing hose.
